New Patients
Please be sure to bring athletic clothing that doesn’t restrict any arm or leg movements. To save you time at our office, you may download and complete our patient history form before your first visit. Please plan to arrive 15 minutes prior to your appointment!
What Do I need?
Your doctor will watch you perform various activities such as walking, stepping off a step, standing on one leg, reaching overhead and others. You may be asked to perform some simple exercises to see how your body stabilizes. The doctor will also extensively assess your joints and muscles to find which ones may need treatment. After discussing what was found on the examination, treatment options will be discussed. If we do not think we can help you or that you will benefit from another specialist, we will refer you to the appropriate one.

Below are the correct directions from the 161 East exit off of I-270:
Once you are heading east on Dublin-Granville Rd (161E), at the first light, turn right (south) onto Frantz Rd. Go through 2 traffic lights. Turn right into the Millennium office park. There is a stone sign with the word Millennium on it by the street. We are in the 3rd building on the right

Wednesday: 12:30pm – 6:30pm / Thursday: 7:30am – 2:00pm
Friday: 7:30am -2:00pm / Saturday: 8:00am-10:00am
